Root Cause Analysis – The Most Damaging Thing For Safety

To quote Greg Smith (WHS Lawyer) from the video below and from the must read book Risky Conversations, The Law, Social Psychology and Risk:

“……..in my view, the notion of root cause and root cause analysis has done more to damage our understanding of safety management than any other single term we use in this industry!”

I’ve just finished reading Risky Conversations, The Law, Social Psychology and Risk and it has completely transformed my understanding of safety, risk, what the law really expects and how different reality is from systems and paperwork. After 30 years in the game I thought I had a pretty good handle on it but how misguided I was!!. The video below is one of 22 that accompany the book and I urge you to take some time out and watch it and learn how to make your investigations more effective.

seek investigationsIf you are totally confident that your existing incident investigation skills and processes are effective then please don’t attend the upcoming SEEK Essentials in Investigations Workshop as it may just dramatically change your thinking about that!
